Practice Exercise 3: Creating RCA and PIR
Create an RCA and a PIR document of the Web App incident encountered.
The RCA and PIR should be uploaded below the filename student_Name_RCA and student_Name_PIR
Task 1: Create an RCA of the WebApp incident to identify the possible root cause of the issue.
- Using the 5 Why Method identify the possible root cause of the web app downtime issue.
Task 2: Create a PIR on the WebApp downtime issue.
- Using the Retrospective Method, create a PIR document filling out/answering the To Do in the template.
- Here are the known factors/information to consider:
- incident was reported by a user September 1, 2023 5:50 PM
- OOM cause was discovered by the tiered support around 8:00 PM
- HTTPD service was started 8:30 PM
- Incident was considered resolved September 1, 2023 9:00 PM
- No Alerting/ Notification System currently in place
- Incident Did not breach SLO with customers
- No system/tool to measure number of user affected currently in place.
- No regular announcement/updates to stakeholders were released during time of outage
Task 3: Upload RCA and PIR for checking.
- Upload created RCA and PIR Document in the drive below with filenames Student_Name_RCA and Student_Name_PIR respectively.
